Salary: $23 - $25

Case Manager

The Cas Manager will advocate for and advise clients withing their assigned caseload. They will develop, monitor, and implement individualized treatment plans in collaboration with the client and the treatment/clinical team members. A strong Case Manager will possess characteristics as highly developed interpersonal communication skills, empathy and compassion, ability to take on multiple tasks and have thick skin.

Clinical Director /Ops Manager / CEO

Duties:

Create and monitor individual treatment plans for
clients

Conduct individual, group, family and substance abuse therapy as needed

Be familiar with community resources

Knowledge and implementation of IMS and ASAM requirements

Have an understanding of current Substance Abuse Regulations

Maintain
current and accurate documentation/ records/ files in accordance with facility policies

Facilitate crisis and therapeutic interventions

Participate in staff and treatment team meetings

Perform other related duties as required and/or assigned

Adhere to policies and procedures

Minimum Qualifications:
M

Must have high school diploma

CPR / First Aid Certification

TB Test (current) with a clearance of physical to work in Behavioral Health Services.

Must be registered with an accredited agency in California

Proficient in software for an EMR, excel and databases

Physical Requirements

Must be able to sit, stand, and walk intermittently. May be required to lift up to 25 lbs. May be required to ascend/descend stairs
