Key Responsibilities:
Work closely with Child Protective Services (CPS), program teams, and families to address mental health needs.
Develop Crisis Management Plans in collaboration with the case manager.
Conduct assessments and establish clinical goals, objectives, and interventions tailored to each family's needs.
Monitor and document weekly case plan progress toward service goals.
Implement individualized treatment interventions based on the case plan.
Assist in developing and coordinating discharge and aftercare plans.
Provide 24/7 crisis intervention support as needed.
Qualifications:
Master's Degree in Social Work, Psychology, Counseling, or a related behavioral health field.
Mental health certification (PCMHT or CMHT) and/or license (LPC, LCSW, LMSW, LMFT, LMFTA) within 30 days of employment.
At least two years of clinical experience working with children, youth with serious emotional disturbances, and families with complex needs.
Valid driver's license, a good driving record, and legally appropriate auto liability insurance.
Willingness to use a personal vehicle for work-related travel.
