Position Summary: We are seeking a Licensed Practical Nurse (LPN) to provide patient-centered care in a home health setting. The LPN will deliver services in accordance with the Connecticut Nurse Practice Act, agency policies, and professional standards of care, under the direction and supervision of a Registered Nurse.
Qualifications:
Graduate of an accredited school of practical nursing
Active and unrestricted LPN license in the State of Connecticut
Minimum one (1) year of experience in home health or similar setting preferred
Valid driver’s license, reliable vehicle, and current auto insurance per state and agency standards
Proficient in documentation, communication, and time management
Strong interpersonal skills and ability to work independently in client homes
Essential Functions:
Provide direct nursing care including physical care, emotional support, and patient education
Administer medications and treatments as prescribed and documented
Accurately complete visit notes and MARs within 24 hours of service
Observe, report, and document changes in patient condition to the RN and physician
Follow infection control, safety practices, and agency procedures
Participate in ongoing staff education and in-service training
Support patients in achieving optimal health outcomes and independence
Communicate effectively with the healthcare team, patients, and families
Use proper EVV clock-in/out procedures for accurate visit tracking
Maintain confidentiality in all aspects of care delivery
Physical/Environmental Demands:
Requires the ability to sit, stand, pivot, twist and change position frequently Requires the ability to lift, push, pull, and carry 20-50 lbs frequently.
Requires the ability to climb stairs, navigate all types of residences and access means.
Requires the ability to utilize telephone, laptop computer and clinical equipment.
Requires the ability to travel to/from Agency, client homes and other professional appointments on a frequent daily basis.
