A respected all-inclusive care program for seniors in Clearfield is hiring a full-time Nurse Practitioner or Physician Assistant.
This role offers the chance to provide thoughtful, long-term care for a stable patient panel, without nights, weekends, or productivity quotas.

*What You'll Do:*
* Conduct exams and manage chronic conditions for older adult participants
* Collaborate with RNs, NPs, and other team members to deliver comprehensive care
* Support care planning, medication management, and diagnostic follow-ups
* Lead quality and safety initiatives across the center
* Provide guidance and mentorship to care team members
*What You'll Need:*
* PA license as a Nurse Practitioner or Physician Assistant
* CPR and First Aid certification
* Valid driver’s license and willingness to travel locally
* Experience with older adult care or a strong interest in the geriatric population
* Willingness to complete an in-depth PACE orientation
