Comfort Home Care is in search of per diem RNs and LPNs in Boston and the surrounding areas. Our nurses visit a diverse population, mostly with mental health diagnoses, performing nursing assessments, communicating with other providers, and maintaining patients in the community as independently as possible.

Day to day responsibilities of a Comfort Home Care Nurse:

• Provide initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
• Initiate the plan of care, necessary revisions, and perform OASIS assessments at appropriate time points when applicable.
• Initiate and perform appropriate nursing interventions.
• Coordinate services and inform physicians and other personnel of changes in the client's condition and needs.
• Collaborate with patient /family and other health care providers and/or community resources with planning of care and discharge.
• Prepare clinical and progress notes and submit all required paperwork in a timely manner according to agency policy and procedure.
