*Job Description: Advanced Hospice RN Clinical Liaison *

*Position Overview:*

Advanced Hospice is seeking an experienced and resilient RN Clinical Liaison to join our fast-growing team. This role is essential in providing high-quality hospice care by ensuring seamless communication and education among patients, families, and healthcare facilities.

*Location*:

Pittsburgh, PA

*Schedule*:

Monday to Friday with some rotating on-call responsibilities.

*Travel Requirements:*

This position requires 100% travel to patient homes, facilities, and hospitals as necessary.

*Responsibilities:*

· Conduct hospice eligibility assessments based on thorough knowledge of Medicare Hospice Regulations.

· Educate patients, families, and facility staff about hospice services and patient-specific care plans.

· Act as a primary point of contact to ensure coordination and continuity of care.

· Utilize strong communication and time management skills to effectively manage multiple cases and priorities.

· Participate in interdisciplinary team meetings to review patient plans and compliance with hospice criteria.

· Provide ongoing support and education on the hospice process to healthcare providers within the community.

*Qualifications:*

· Must possess an active RN license.

· Proven hospice experience with a deep understanding of Medicare regulations and eligibility requirements for hospice care.

· Exceptional interpersonal, communication, and presentation skills.

· Robust organizational and time management skills are essential.

· Must be adaptable, patient-focused, and capable of handling challenging situations with tact and empathy.
