LPN for Helen Porter Rehab & Nursing: Every other weekend, floating required. Cat on unit - cannot be allergic. Questionnaire attached to position files needs to be reviewed with interested candidates and uploaded in system for IVS review. Requirements: • Dementia capable care certification preferred • A minimum of 1 year of experience • Prior travel required • Questionnaire attached to position files needs to be reviewed with interested candidates and uploaded in system for IVS review • EPIC experience from within the past 2 years is required and nonnegotiable Additional Information: • IVS to OFFER • Monday to Sunday • Bi-weekly Orientations on Monday • No block schedules for any reason • This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit • Please ensure all employment gaps over 30 days are explained • Covid and Flu declinations accepted- please have either proof or declinations in packet Disclaimers: • Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended • Vehicles should be equipped with all season or snow tires during the months of November – April
